Early Identification of Cognitive Impairment

Grant Description:

This 12-month pilot implements and evaluates the impact of adding an evidence-based cognitive screening tool to other assessment tools being used by two existing in-home service programs for older adults in Onondaga County. The intent is (a) to have a mechanism for the early identification of elders with previously undetected cognitive impairment and referral for a comprehensive cognition screening; and (b) to increase their access to needed health and community support services. Results from the evaluation of this pilot will be used to justify and apply for additional funding to expand the program within the Central New York region and ultimately to other CEADs and Offices for Aging in New York State.
